年菜推薦宅配一位來自台中的41歲婦女,多年來飽受不孕之苦,因為子宮內膜沾黏,未達更年期就停經達數月,在醫生努力下,以微創性子宮中秋烤肉鏡手術治療,重建子宮內膜,不到半年就懷孕。中國醫藥大學附設醫院生殖醫學中心主任何彥秉表示,微創性子宮鏡治療的範圍包括子宮肌瘤、子宮息肉、子宮內膜沾黏等,特色是不留傷口,恢復期短,針對不孕患者,為不錯的治療方式。

何彥秉指出,造成這位婦女不孕的原因是骨盆腔發炎導致子宮沾黏,過去雖然曾在他處接受子宮擴張術和避孕器置放,卻沒有改善沾黏情形,月經也停止。但患者並非更年期停經,卵巢功能仍正常,因此要治療不孕,必須打開幾乎全部沾黏的子宮腔。透過子宮鏡搜尋,成功在患者的右側輸卵管附近,找到一塊仍正常的子宮內膜,運用水球擴張搭配藥物治療,順利完成重建工作。婦女在手術後,月經周期恢復,不到半年便懷孕。

但這位患者懷孕之路一波三折,在剖腹產後出現胎盤和子宮沾黏,經由醫師緊急措施搶救,順利保留生機。何彥秉說,這類患者一旦受孕,發生前置胎盤或植入性胎盤,導致產前或產後出血的風險較一般人高出許多,因此生產前後必須特別留意。

至於成功治療不孕的微創性內視鏡手術,主要是利用天然孔洞的內視鏡,操作時由陰道進入,全程不會留下傷口,復原速度快。過去沿用多年的腹腔鏡,需要在腹部開出一個小傷口,相湖口 火鍋推薦較之下,微創手術對患者負擔更少。目前包括子宮肌瘤、子宮息肉、子宮內沾黏都可利用微創性內視鏡手術治療,而合併多重因素的不孕患者,在進行試管嬰兒前,若先用子宮鏡清除子宮內沾黏,也可提高受孕機會。
